Life & Disability

Life and Disability insurance, through Lincoln Financial, provides financial security to you and your family if you pass away or become unable to work.

Basic Life Insurance

As an eligible employee, you receive Basic Life insurance equal to 1.5 times your annual earnings, to a maximum of $1,000,000. Basic Life is provided by the company at no cost to you. You can choose to cap your Basic Life benefit at $50,000.

Voluntary Life Insurance

In addition to your Basic Life coverage, you may buy Voluntary Life coverage at discounted rates. The chart describes the amounts of coverage you can buy for yourself, your spouse, and your child(ren).

*Evidence of Insurability (EOI) may be required beyond the guaranteed issue amount or outside of your initial benefits eligibility period.
**Basic Annual Earnings excludes overtime for Class 1 and includes overtime for Class 3; commissions and bonuses are excluded for both. The 5x maximum is rounded to the next higher $10,000 for Class 1 and to the nearest $10,000 for Class 3.

***The 50% spouse limit is rounded to the next higher $5,000 for Class 1 and to the nearest $5,000 for Class 3. Dependent coverage requires the employee to be enrolled in Voluntary Life. 

Note: Employee and spouse coverage reduce by 33% of the original amount at age 70 and an additional 33% at age 75; coverage ends at retirement.

Beneficiary Basics

The person(s) you select as your beneficiary(ies) will receive your coverage amount, in the event of your death.

  • Choose your beneficiary at big5employeeportal.com.
  • Review your beneficiary designation periodically to ensure it reflects your current wishes.
  • Change your beneficiary anytime at big5employeeportal.com or call 844.396.5948.
  • Beneficiaries should be over the age of 18.

Voluntary Short-Term Disability (STD)

STD coverage provides you with partial income replacement if you are unable to work due to a non-work-related illness or injury. You can purchase Voluntary STD coverage through payroll deductions.*

Voluntary Long-Term Disability (LTD)

LTD pays you a portion of your earnings if you cannot work for an extended period due to a disabling illness or injury. You can purchase Voluntary LTD coverage through payroll deductions. Big 5 pays half the cost of premiums for this benefit.

You will continue to receive benefits as long as you meet the definition of disability or until you reach Social Security Normal Retirement Age. Benefits are reduced by other sources of disability income you may qualify for, such as Social Security and Workers’ Compensation.

Report Absence Online

To report your absence quickly and securely online, visit LincolnFinancial.com and follow these five steps:

  1. Log in to LincolnFinancial.com.
    • First-time users must register using company code: BIG5CORP
  2. Select Report a New Claim or Leave and answer a few questions about yourself and your absence.
    • You’ll need to provide your employee identification number or another ID required by your employer, the dates of your absence, and your diagnosis and physician information, if applicable.
  3. Choose Submit.
    • If you’re filing a disability claim, you’ll need to download, sign, and submit a medical authorization form to send to your doctor.
  4. Keep a record of your claim or leave number.
    • Print your personal report, which includes your claim or leave number and a summary of the claim details.
  5. Check the status of your claim or leave request online at LincolnFinancial.com.
    • You can also opt into text messaging at LincolnFinancial.com to receive notifications about your absence, such as the receipt, approval, and extension of an absence, benefit payment information, and the closure or extension of a claim due to return to work.
